Please bring your physician’s signed prescription for therapy and any forms you printed from the website completely filled out, as well as your insurance card and picture ID.
Please wear comfortable clothing. Most people prefer gym attire.
Most sessions last about an hour. Your first appointment may take some additional time.
Strive! is contracted with many managed care companies. We also can treat patients as out of network if your insurance company allows it. Just give us a call and we will check for you.
Our clinics hours vary. Most offer appointments as early as 7:00 am and as late as 6:00pm. We have also been known to work through a lunch hour in order to accommodate our patients’ needs.
Our front office representatives can answer most billing questions, however if they cannot, you may call 352-351-8883 for additional assistance.
It depends on the tissue/structure involved, however most patients start to notice a difference in 1-2 weeks.

That’s a question we get a lot. In almost every case, we can proudly answer, “You bet.” That’s because we participate with 98% of insurance companies. We can also obtain authorization and verify your coverage before your first visit – so there won’t be any surprises once you start your recovery program.

Did you know that you can be evaluated and treated by a licensed Physical Therapist without a referral/prescription from your physician? This is referred to as Direct Access. Many insurance plans pay for services under Direct Access, does yours? Call the location most convenient for you and we will verify your coverage and review your options with you.
